Key Responsibilities:
Client Care: Ensure smooth introductions and positive experiences for clients and their families by responding promptly to inquiries and conducting in-person nursing assessments.
Care Plan Development: Create personalized care plans, match caregivers with clients, and provide clear orientation to caregivers on client needs.
Ongoing Communication: Maintain regular contact with clients, families, and caregivers to monitor and revise care plans as needed.
Community Outreach: Build strong relationships with local referral sources and become a key advocate for quality home care services.
You'll also have the chance to be creative and proactive in promoting services that make a real difference to the people who rely on them.
Qualifications:
Bachelor's or Associate's Degree in Nursing from an accredited institution.
Active Registered Nurse (RN) license in Maryland.
Minimum of 2 years of nursing experience.
Case management experience is a plus, though not required.
Ability to travel within Baltimore County, Harford County, and Baltimore City for client visits.
COVID-19 vaccination required (exemptions may be requested for medical or religious reasons).
