As the
Care Coordinator , you will provide support to patients by working with the treatment team and other recovery support staff to identify needs and facilitate connections to resources, supports, and services available in the community. You will maintain close communication with all involved parties for minimal interruptions in care. Also, you will work with patients to orient them to the program and identify their own individualized recovery goals. In addition, you will also ensure patients are receiving holistic support and that all parties involved in patient care are cohesively collaborating.

Qualifications Qualifications - Bachelor’s degree in behavioral health sciences is required.
- VA: Bachelor’s Degree (including QMHP-A) and either one year of direct training/experience with substance disorders, or minimum one year of clinical experience/training with co-occurring diagnoses (SUD and mental illness); or a Board of Counseling CSAC
- Thorough understanding of the Relapse Models and Harm Reduction concepts.
- Working understanding of the unique needs/dynamics of the patient population.
- Knowledge of community resources, networking, and referral processes.
- Experience in a Case Management, or Care Coordination setting.
- Knowledge of and proven experience navigating and documenting in an EMR system.
- This position is subject to a background check for any convictions directly related to its duties and responsibilities.
