Job Description

As RN, Nurse Home Care Case Manager, you oversee the nursing objectives of Caring for Family of Companies. You're responsible for Client Care, Nursing-related Caregiver training, and professional development You ensure the highest quality of care for Clients and maintain strict compliance with established protocols and In Home Care licensing standards. This position reports to the Director of Nursing Services.

Responsibilities :

• Oversee nursing services, including medication administration program, and ensure standard adherence.
• Maintain Client confidentiality beyond HIPPA-appropriate communication with doctors and family as required.
• Review medication administration and error reports to maintain safety and compliance protocols.
• Conduct scheduled Nursing and medication evaluation and assessment visits, addressing Client concerns.
• Review medication orders to confirm Client needs and contradictions, collaborating with providers for adjustments when needed.
• Travel to various branches to fill staffing gaps, perform nursing assessments, and train on medication administration.
• Train Caregivers on medication administration and Nurse delegated duties, ensuring competency and ability to deliver care.

Portland Oregon Metro Service Area – including Portland, Banks, Beaverton, Hillsboro, Cedar Mill, West Lynn, Lake Oswego, Tigard, Gresham, Milwaukie, Oregon City, Clackamas, Happy Valley 

Requirements

• Excellent verbal and written communication in English

• Proficiency in the Microsoft Office Suite

• Strong focus on accurate record-keeping and time management

• Thorough understanding of the protection and use of Protected Health Information (PHI) following HIPPA guidelines.

• Ability to take the initiative, work independently, and thrive in fast-paced team environments with a collaborative spirit.
