What You'll Do:
Provide Compassionate Care: Conduct thorough patient assessments and collaborate with a supportive care team to develop and implement personalized care plans.
Coordinate Care: Make referrals to other hospice care specialists as needed, ensuring patients receive comprehensive support.
Maintain Accurate Records: Develop, prepare, and maintain detailed patient care progress records, documenting care with accuracy and timeliness.
Manage Symptoms: Document pain and symptom management changes and outcomes, ensuring patient comfort and well-being.
Educate and Empower: Teach patients, families, and caregivers valuable patient care techniques, empowering them to actively participate in the care process.
Communicate Effectively: Regularly communicate with the clinical manager and care team, fostering seamless collaboration.
Provide On-Call Support: Share on-call responsibilities, providing vital support to patients and families as needed.
What You'll Bring:
One year of nursing experience.
Active and unrestricted RN license in Pennsylvania.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.
