Are you an experienced and compassionate Registered Nurse looking to make a real impact in the lives of individuals in your community? We are seeking dedicated Care Managers (RN) to join our team in the Toledo, OH area. In this role, you will conduct field visits and provide care coordination for individuals of all ages, ensuring they receive the support they need to remain in their homes safely. This is a remote position that requires travel within the Toledo area.

Key Responsibilities
Conduct in-person field visits to assess members' well-being and coordinate care plans.
Manage a caseload that includes both individuals over and under the age of 60.
Maintain accurate and timely documentation within an EMR system.
Collaborate with team members through daily group chats and weekly meetings to discuss updates and challenges.
Prioritize tasks efficiently while ensuring compliance with healthcare standards.
Work Schedule
Monday - Friday, 8:00 AM - 5:00 PM
No overtime required
What You'll Be a Part Of
A supportive and collaborative team of 13 Case Managers who work together to provide high-quality care.
Weekly team calls to discuss cases and challenges, ensuring continuous learning and support.
A mission-driven team focused on delivering essential healthcare services to vulnerable populations.
Key Projects & Purpose
This team plays a vital role in implementing a care program designed to support individuals in Ohio who need assistance staying in their homes. By coordinating care under a structured healthcare contract, the team helps ensure that members receive the services they need in a timely and effective manner.
What We're Looking For
Education & Certification
Required: Associate's degree in Nursing (ADN)
Required: Active RN license in Ohio or Compact License
Experience
Must have prior experience in home health care. Knowledge of waiver programs is strongly preferred over hospital experience.
Ability to work independently, manage a caseload, and ensure all documentation is completed accurately and on time.
Skills & Competencies
Proficiency in electronic medical records (EMR) systems and other healthcare-related technology.
Strong time management and organizational skills.
Self-motivated with the ability to work independently in a remote setting.
Performance Expectations
Meet documentation requirements and complete visits as scheduled.
Regularly review reports to ensure timely and accurate member assessments.
Compensation & Contract Details
Pay Rate: $40/hr
Contract Length: 3 months (potential for extension or full-time hire)
Maximum Bill Rate: $46/hr
