Objective/Overview

The clinic is known for providing high quality care to diverse patients and is moving forward in improving timely access to care for patients.  The Integrated Behavioral Health psychologist plays a key role in providing mental health and behavioral interventions within both the primary care and dental clinics.  This role focuses on short-term, evidence-based interventions that promote mental health and overall well-being. The clinician will collaborate with primary care providers, medical staff, and other healthcare professionals (i.e., clinical pharmacist, nutritionist, community health workers, care coordinators, dentist, and dental hygienist) to support comprehensive, patient-centered care, addressing the physical, emotional, psychological, social and cultural needs of patients. The clinician will focus on addressing mental health concerns, medication adherence, lifestyle changes for chronic health conditions, smoking cessation, and behavioral health issues, providing comprehensive care across medical and dental settings.

Work Environment
The primary work setting is the clinical office at CUHCC, with approximately 60% of the role (three days per week) conducted in person within the primary care clinic and an additional 40% (one day per week) performed in person within CUHCCâ™s outpatient therapy clinic. This role may involve contact with patients who have contagious illnesses.
Remote telemedicine work (up to one day per week) may be available depending on patient needs and productivity expectations.

Reporting Relationship
This position reports to the Integrated Behavioral Health Program Manager and has no direct reports.

Key Responsibilities
Integrated Behavioral Health Consultation (60%)

  • Conduct brief behavioral health assessments and provide short-term interventions for patients with a range of mental health and substance use concerns in both the primary care and dental settings.
  • Conduct risk assessments for patients who may pose a risk to themselves or others.
  • Consult with medical providers for walk-in patients with mental health or substance use concerns
    Develop and implement individualized care plans in collaboration with medical and dental providers, tracking progress toward measurable goals.
  • Facilitate group therapy for patients referred for mental health services, addressing common mental health conditions such as stress management, anxiety, depression, and coping with chronic illnesses.
  • Lead psychoeducational groups that promote understanding of mental health conditions and support behavioral change.
  • Co-visit with medical and dental providers as well as allied staff, such as clinical pharmacists and nutritionists, to offer integrated support for patients with complex needs:
  • Smoking cessation: Collaborate with clinical pharmacists and dental providers to address smoking cessation using behavioral strategies and pharmacological support.
  • Medication adherence: Partner with clinical pharmacists to help patients improve medication adherence for mental and physical health conditions.
  • Lifestyle modifications: Work with nutritionists and dental professionals to promote lifestyle changes for patients with chronic conditions (e.g., diabetes, hypertension), integrating behavioral health strategies to support sustainable behavior change.
  • Collaboration and Coordination:
    • Work closely with primary care and dental providers, clinical pharmacists, nutritionists, and other allied staff to identify patients who would benefit from behavioral health services.
    • Participate in interdisciplinary care team meetings to discuss patient progress, coordinate care plans, and ensure integrated, whole-person care across both medical and dental services.
    • Facilitate referrals to community mental health services or specialists for patients requiring more intensive or long-term care, ensuring seamless transitions.
      Review medical provider schedules and patient charts for opportunities for behavioral health interventions, and conduct pre-visit planning meetings with medical providers and certified medical assistants
  • Documentation and Compliance:
    • Maintain accurate and timely documentation in electronic health records (EHR) across primary care and dental clinics, including patient assessments, treatment plans, co-visit notes, and group therapy outcomes.
    • Ensure compliance with clinic policies, state and federal regulations, and professional standards for integrated mental and physical healthcare.
    • Provide therapy and assessment services as a Mental Health Professional:  (35%)
      Assess, diagnose and treat patients utilizing specialized knowledge and skills, develop patient care plans based on defined area of clinical expertise, risk and overall patient care complexity.
    • Conduct diagnostic assessments, treatment planning, individual, group, couple and family therapy interventions. 
    • Prepare and complete mandated documentation within established timelines including diagnostic assessments, individual treatment plans, progress notes, discharge summaries, safety assessments, and required screenings (CASII/ECSII, PHQ-9, CAGE, etc.).  
    • Conduct psychological assessments as needed, including brief neuropsychological evaluations to assess cognitive functions, memory, attention, and executive functioning, as well as psychosocial and personality assessments to inform treatment planning.

Education and Training (5%)

  • Provide clinical supervision for clinical trainees and mental health students on field practicum rotations as required
  • Complete required continuing education to maintain professional licensure
  • Provide and assist with interprofessional trainings on behavioral health topics

Required Qualifications

  • Independently licensed or license eligible Mental health Professional as defined by MN statute as a Licensed psychologist.
  • Advanced degree plus four years of experience or a terminal degree plus 1 year of experience.

Preferred Qualifications

  • Two to three years of experience in providing clinical services to immigrant, refugee or other underserved populations.
  • Two to three years of experience providing integrated behavioral health setting, especially in a primary care clinic.
  • Fluency in Spanish, Somali, or any South East Asian language.
  • Two to three years of experience and training in Trauma Informed Care.
