Job Description

Human and Social Service Assistant needed for our government client in Randallstown. The Human and Social Service Assistant will provide case management services to the senior and disabled population. One year contract position with possible extension or contract to hire consideration. The non-negotiable pay rate is $19.00 per hour.

Essential Duties:

  • Provides case management services to the senior and disabled population who are in need of healthcare services.
  • Will provide regular visits in person, by telephone and/or virtual to residents.
  • Update Social Worker staff on any health changes relating to overall care.
  • Reviews client files to ensure that all documents are signed and completed per contract compliance.
  • Receives and routes incoming mail; may deliver mail to Post Office and maintain mail log.


Qualifications:

  • At least one year of case management experience to include long term care planning and patient monitoring are required.
  • Associate's or Bachelor's degree in Human Services, Behavioral Health, Psychology or Public Health Administration in addition to one year of case management experience working with the senior or disabled population is required.
  • Must be able to complete the Maryland Department of Health required training requirements for "New Supports Planners.
  • Must be able to learn the operations of Maryland Long Term Services & Supports (LTSS) computer system.
  • Must have proficient Microsoft Word and Excel skills.
  • Must be able to work from 8:30 am-4:30 pm Monday through Friday with a 60 minute non paid lunch.
  • All work is performed on site. Client will evaluate a hybrid work schedule after six months of performance.
  • Must have a valid driver's license, good driving record and own vehicle.
  • Must be able to pass a background check and drug test.
