About the Role:
As a Hospice LPN, you'll administer skilled nursing care under the supervision of a registered nurse, working with patients of all ages in their homes. You'll coordinate care with the interdisciplinary team, patients, families, and referring agencies.
What You'll Do:
Under the direction of the RN, help identify patients' physical, psychosocial, and environmental needs.
Participate in care planning and implementation with the RN, following the plan of care.
Provide care using infection control measures that protect both staff and patients.
Ensure continuity of care by delivering high-quality patient care.
Communicate effectively with patients, families, team members, and other healthcare professionals.
Monitor assigned cases to ensure compliance with payer requirements.
Demonstrate commitment, professional growth, and competency.
Uphold agency philosophy and policies to ensure quality of care.
What You'll Need:
Graduate of an accredited school of vocational nursing.
One year of experience as a Licensed Vocational Nurse in a clinical care setting is required; home health experience is a plus!
Strong nursing skills aligned with generally accepted standards of practice.
Excellent interpersonal skills.
Reliable transportation is essential for this on-the-road role.
Current CPR certification and Hepatitis profile.
Ability to work a flexible schedule, including some on-call responsibilities.
Ability to travel locally.
Environmental and Working Conditions:
Work occurs in patients' homes, with varying conditions.
Potential exposure to blood, bodily fluids, and infectious diseases.
Some exposure to inclement weather.
Schedule:
Full-time, Monday through Friday, with potential for overtime and on-call hours.
