About the Role:
We're looking for a dedicated LPN to join our team and provide skilled nursing care to patients of all ages in the comfort of their own homes. You'll work under the supervision of a Registered Nurse (RN), collaborating closely with the interdisciplinary team, patients, families, and referring agencies to deliver comprehensive and compassionate care. This role offers a chance to build meaningful relationships with patients and families, providing essential support and comfort during a challenging time. This is a full-time position offering a competitive salary and benefits package. While this is a field-based role requiring travel, occasional meetings at alternate locations are possible.
What You'll Do:
Assist with assessments: Under the direction of the RN, help identify the patient's physical, psychosocial, and environmental needs through documentation, clinical records, case conferences, team reports, and evaluations.
Participate in care planning: Collaborate with the RN to plan and implement patient care in accordance with the established plan of care.
Provide skilled care: Deliver skilled nursing care while adhering to infection control measures to protect both staff and patients.
Ensure continuity of care: Provide high-quality patient care to maintain continuity and consistency.
Communicate effectively: Communicate effectively with patients, families, team members, and other healthcare professionals through clinical notes, case conferences, and evaluations.
Monitor cases: Monitor assigned cases to ensure compliance with requirements.
Demonstrate professionalism: Demonstrate commitment to professional growth and competency.
Promote quality care: Uphold and promote our philosophy and administrative policies to ensure the highest quality of care.
What You'll Bring:
Graduation from an accredited school of vocational nursing.
One year of experience as a Licensed Vocational Nurse in a clinical care setting (home health experience is a plus!).
Strong nursing skills that meet generally accepted standards of practice.
Excellent interpersonal and communication skills.
Reliable transportation.
Current CPR certification.
Hepatitis profile.
Ability to work a flexible schedule, including possible on-call responsibilities and overtime.
Ability to travel locally.
