We are seeking a dedicated Hospice LPN to join our team. What You'll Do:
Deliver Skilled Nursing Care: Administer skilled nursing care to clients of all ages in their homes, under the supervision of a Registered Nurse.
Collaborate and Coordinate: Coordinate care with the interdisciplinary team, patients and their families, and the referring agency.
Assess Patient Needs: Assist the RN in identifying patients' physical, psychosocial, and environmental needs through documentation, clinical records, case conferences, team reports, and evaluations.
Implement Care Plans: Participate in planning and implementing care in conjunction with the RN, following the established plan of care.
Ensure Quality Care: Provide care utilizing infection control measures that protect both staff and patients.
Communicate Effectively: Communicate effectively with patients/families, team members, and other healthcare professionals through clinical notes, case conferences, and communication notes.
Monitor Cases: Monitor assigned cases to ensure compliance with requirements.
Promote Quality Care: Demonstrate commitment, professional growth, and competency, promoting the philosophy and policies to ensure the highest quality of care.
What You'll Need:
Graduate of an accredited school of vocational nursing.
One year of experience as a Licensed Practical Nurse in a clinical care setting (home health experience preferred).
Strong nursing skills as defined by generally accepted standards of practice.
Excellent interpersonal skills.
Reliable transportation is essential for this role, as travel to patients' homes is required.
