Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Kitsap Lake, Washington. In this hybrid role, you will conduct in-person preadmission evaluations and assessments throughout Kitsap Lake and its surrounding areas.

As a Clinical Evaluator, you will perform face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals in need. Through these essential evaluations, you’ll play a key role in promoting well-being, enhancing quality of life, and advocating for services that make a lasting difference in the communities we serve.

This full-time Clinical Evaluator position offers both flexibility and purpose—combining remote work from your home office with local travel. Acentra Health covers and reimburses all travel-related expenses, including mileage and parking. In addition, Acentra provides all necessary technology equipment for your home office, including a laptop for use in the field.

Job Responsibilities:

  • As a Clinical Evaluator. you will be conducting PASRR Level II assessments and evaluations, developing informed level-of-care recommendations based on your assessments.
  • Complete accurate and thorough PASRR reviews by evaluating patients’ physical, behavioral, and psychological functioning to determine the appropriate level of care. 

  • Assess and analyze patients' psychological, emotional, behavioral, and physical functioning to inform clinical decisions.
  • Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations.
  • Provide well-informed level of care recommendations based on clinical evaluations, ensuring adherence to established workflows, including re-evaluation of records in response to psychiatric input.
  • Ensure compliance with all contractual and regulatory guidelines when formulating level of care recommendations.
  • Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
  • Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
Qualifications:

Required Qualifications and Experience

  • Must possess an active, unrestricted license in the State of Washington in a healthcare or related field, such as Licensed Psychologist (LP), Licensed Professional Clinical Counselor (LPCC), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Professional (LMHP), Registered Nurse (RN) and Psychiatric Mental Health Nurse Practitioner (PMHNP). 
  • Graduation from an accredited college with a relevant degree program and a minimum of 2 years of experience in assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia..
  • Familiarity with Preadmission Screening and Resident Review (PASRR) requirements per State and Federal Regulation.
  • Knowledge of public mental health systems and medical programs, including public sector services and supports, with a strong understanding of Level of Care guidelines and related standards of practice.

  • Expertise in assessing and analyzing psychological, emotional, behavioral, and physical functioning to inform clinical evaluations.

  • Experience formulating clinical diagnoses for complex psychological and behavioral health conditions.

  • Knowledge of Level of Care standards and best practices related to mental health, geriatrics, and long-term care needs.

  • Effective communication skills with the ability to interact effectively with individuals and groups through verbal, written, and electronic formats.
  • Proficient in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).
  • Experience diagnosing psychiatric disorders using current DSM standards (state-specific) is a plus.

  • Previous experience working with the geriatric population is preferred.

Compensation

The compensation for this role is $85,280 to $91,520 .00 base salary 

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
