Key Responsibilities:

• Deliver comprehensive medical care to residents in a long-term care setting
• Collaborate with physicians, nursing staff, and other healthcare professionals to ensure optimal patient outcomes
• Develop and implement individualized treatment plans
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on health conditions, medications, and wellness strategies
• Maintain accurate and up-to-date medical records

Qualifications:

Required:

• Nurse Practitioner Certification (Adult or Family)
• Active NYS License
• Board Certification
